ACKNOWLEDGEMENT OF POLICIES
By scheduling an appointment at our clinic, clients acknowledge and agree to adhere to the policies outlined.
These policies are subject to change without prior notice. We appreciate your cooperation and understanding in ensuring a smooth and enjoyable experience for all clients and staff.
APPOINTMENT SCHEDULING AND CANCELLATION
All appointments must be scheduled in advance either by phone, online, or in-person. We require a 24-hour cancellation notice for all appointments. Failure to provide this notice will result in a cancellation fee equal to 50% of the service fee. In the case of no-shows, clients will be charged the full amount of the service fee. Late arrivals may result in a shorter appointment or rescheduling, depending on therapist availability. Late clients will still be charged the full service fee.
NEW CLIENT PROCEDURES
New clients are required to arrive at least 15 minutes prior to their scheduled appointment time to complete necessary paperwork. Please provide valid identification and insurance information upon arrival. New clients are encouraged to disclose any medical conditions, allergies, or preferences that may affect their treatment.
DIRECT BILLING AND INSURANCE
We offer direct billing to most extended health benefit providers. Clients are responsible for providing accurate insurance information and ensuring their coverage is up-to-date. In cases where direct billing is not possible due to coordination of benefit rejections or other issues, clients will be required to pay for services rendered and submit claims to their insurance providers independently. Clients are responsible for any outstanding fees not covered by their insurance.
INFRARED SAUNA USAGE
Our infrared sauna is available to clients at no additional cost. Sauna usage is limited to a maximum of 30 minutes per session and is to be used prior to treatment, not after. Clients are required to follow safety guidelines and respect other clients' time by adhering to the session duration.
CLIENT CONDUCT AND ETIQUETTE
Clients are expected to treat staff and other clients with respect and maintain a peaceful atmosphere in the clinic. Clients should maintain personal hygiene and arrive at appointments clean and appropriately dressed. Inappropriate behavior or harassment will not be tolerated and may result in immediate termination of services and a ban from the clinic.
CONFIDENTIALITY AND PRIVACY
We are committed to protecting clients' privacy and maintaining confidentiality. All client information will be handled in accordance with applicable privacy laws and regulations. Clients' personal and medical information will only be shared with the necessary staff members and insurance providers, as required for billing and treatment purposes.
PAYMENT AND REFUND POLICY
Payment for services is due upon completion of each appointment. We accept cash, credit, and debit cards. In the event of a dispute or dissatisfaction with services, clients should contact the clinic manager to discuss the issue and explore potential resolutions. Refunds will be provided at the discretion of the clinic.
HEALTH AND SAFETY PROTOCOLS
Clients must adhere to any health and safety protocols currently in place, such as wearing masks or maintaining social distancing. If a client is experiencing symptoms of illness, they are requested to reschedule their appointment to protect the health of staff and other clients. Hand sanitizer and other hygiene supplies are available throughout the clinic and should be used as needed.
AGE RESTRICTIONS AND MINOR CLIENTS
Clients under the age of 8 must be accompanied by a parent or guardian during their appointments. Parental or guardian consent is required for treatments involving clients under the age of 14.
PERSONAL BELONGINGS AND LOST ITEMS
Clients are responsible for the safety and security of their personal belongings while at the clinic. The clinic is not responsible for any lost, stolen, or damaged items.
GIFT CERTIFICATES AND PROMOTIONS
Gift certificates are available for purchase and can be redeemed for any of our services. They must be presented at the time of the appointment and are non-refundable. Promotions and discounts cannot be combined and are subject to their respective terms and conditions.
CELL PHONE AND ELECTRONIC DEVICES
Clients are kindly requested to silence or switch off their cell phones and electronic devices during treatments to maintain a peaceful environment.
