For clients who have already been diagnosed with a WAD (Whiplash Associated Disorders) injury, we require your AB1 and AB2 forms completed by your physiotherapist or chiropractor. These documents are essential for us to provide you with the appropriate treatment planning and to set up Motor Vehicle Insurance Billing so you don't have to pay out of pocket.
